Job summary

Pathways Healthcare seeks an experienced registered nurse to join our physician- and nurse practitioner-led home health and hospice team in Massachusetts. You will assess patients, develop and implement care plans, and coordinate with physicians to optimize outcomes while engaging families in the treatment process.

You will provide in-home nursing care, document clinical notes, and participate in quality improvement activities, with a Monday–Friday schedule and opportunities for professional

Qualifications

• Nursing degree from an accredited program and active license.
• Proficiency with electronic medical records (EMR).
• Excellent bedside manner and clear communication with patients and families.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Must possess a valid driver's license and reliable vehicle.

Responsibilities

• Assesses and evaluates patient status; writes and initiates plan of care.
• Regularly re-evaluates patient and family needs; revises plan of care as needed.
• Develops goals-based care plans; involves patient and family.
• Administers prescribed medications and treatments per physician plan.
• Maintains accurate clinical notes; tracks pain and symptom changes.
• Communicates with physicians about patient needs and changes.
• Ensures patient safety and attends interdisciplinary meetings.
• Participates in on-call duties and coordinates equipment/services.

Position Details:

• Main Office location: Concord, MA
• Service Area: Marlborough, Hudson, Stow, Maynard, Bolton and Sudbury areas.
• Job Type: Full Time
• Schedule: Monday to Friday

Responsibilities:

Patient Care:

• Assesses and evaluates patient’s status by:
• Writing and initiating plan of care.
• Regularly re-evaluating patient and family/caregiver needs.
• Participating in revising the plan of care as necessary.
• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
• Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate.
• Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
• Provides and maintains a safe environment for the patient.
• Attends interdisciplinary group meetings.

Additional Duties:

• Participates in on-call duties.
• Ensures that arrangements for equipment and other necessary items and services are available.
• Supervises ancillary personnel and delegates responsibilities when required.
• Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
• Fulfills the obligation of requested and/or accepted case assignments.
• Actively participates in quality assessment performance improvement teams and activities.

Qualifications:

• 2+ years' nursing experience, preferably in geriatrics is desirable.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

• Salary Range: $72K-90K Annual Salary
