Pathways Healthcare, Llc is seeking a dedicated nurse for in-home health and hospice services in the Worcester area. Join a physician- and NP-led team delivering patient-centered care with a focus on meaningful recovery at home.

The role emphasizes assessment, care planning, and family involvement, with a Monday–Friday day shift and on-call potential. A nursing degree and 2+ years of experience are required, along with an active CPR certification and reliable transportation.

Qualifications

• 2+ years nursing experience, preferably in geriatrics.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable vehicle.

Responsibilities

• Assesses and evaluates patient status by writing and initiating plan of care.
• Regularly re-evaluating patient and family needs.
• Revising the plan of care as necessary.
• Develops a care plan with goals based on nursing diagnosis and family involvement.
• Administers medications and treatments as prescribed by physician.
• Completes and submits clinical notes; tracks pain/symptom changes.
• Communicates with physician and follows orders.
• Maintains a safe patient environment and attends meetings.
• Participates in on-call duties and coordinates equipment/services.
• Supervises ancillary personnel and pursues ongoing education.

Responsibilities:

• Assesses and evaluates patient’s status by:
• Writing and initiating plan of care.
• Regularly re-evaluating patient and family/caregiver needs.
• Participating in revising the plan of care as necessary.
• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
• Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate.
• Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
• Provides and maintains a safe environment for the patient.
• Attends interdisciplinary group meetings.

Additional Duties:

• Participates in on-call duties.
• Ensures that arrangements for equipment and other necessary items and services are available.
• Supervises ancillary personnel and delegates responsibilities when required.
• Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
• Fulfills the obligation of requested and/or accepted case assignments.
• Actively participates in quality assessment performance improvement teams and activities.
