Job Responsibilities:

• Provide clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care, and coordinate with the healthcare team.
• Conduct initial and comprehensive nursing evaluations, develop and update the plan of care, and document accurately and timely.
• Regularly evaluate and re-evaluate the patient's nursing needs, at least every 60 days or as condition changes.
• Perform patient assessments and develop appropriate plans of care in collaboration with physicians.
• Ensure patients meet home health eligibility and medical necessity guidelines.
• Collaborate with physicians and healthcare professionals to initiate and revise care plans.

License Requirements:

• Current RN licensure in the state of practice
• Current CPR certification
• Valid Driver's License, vehicle insurance, and access to reliable transportation
