Job summary

First Choice Home Health & Hospice in Boise seeks an experienced RN Case Manager to deliver in-home skilled nursing care and to manage individualized care plans for patients. You will coordinate with physicians and the care team to ensure comprehensive, compassionate services.

Qualifications

• Active RN license in Idaho or multi-state licensure.
• Home health experience preferred.
• Familiarity with HCHB EMR and OASIS.
• Strong communication and time-management skills.

Responsibilities

• Valid driver’s license and reliable transportation
