Boost Home Healthcare - Framingham-Norwood is seeking a compassionate Registered Nurse to join as a Home Health Case Manager. You will build relationships with patients, provide clinical oversight, and ensure care is right-sized and timely in the home setting.

You’ll conduct OASIS assessments, develop care plans with the team, coordinate with physicians and therapists, maintain Medicare-compliant documentation, and advocate for patients and families while balancing autonomy and work-life balance.

Qualifications

• Active RN license in state of MA; 1 year home health or case management experience preferred.
• Strong critical thinking, communication, and time management skills.
• Ability to work independently and manage a caseload.
• Experience with EHR systems (Kantime/Axxess/HCHB/WellSky) is a plus.

Responsibilities

• Manage the care journey for a caseload of home health patients, coordinating services and ensuring timely care.
• Conduct comprehensive OASIS assessments and develop individualized care plans with the interdisciplinary team.
• Coordinate with physicians, nurses, therapists, social workers, and home health aides to deliver care.
• Monitor patient progress, update care plans, and communicate with families as needed.
• Maintain Medicare-compliant clinical documentation and agency policy.
• Advocate for patients by providing education, resources, and support.

What You'll Be Doing

• Manage the Care Journey: Serve as the primary point of contact for a caseload of home health patients, ensuring seamless coordination of services.

• Documentation: Maintain accurate and timely clinical documentation in accordance with Medicare guidelines and agency policy.

What We’re Looking For

• License: RN (Registered Nurse) with active (state) license.
• Experience: At least 1 year of experience in home health or case management preferred.
• Skills: Strong critical thinking, communication, time management, and the ability to work independently.
• Tech-Savvy: Comfortable using EHR systems (experience with Kantime, Axxess, HCHB, or WellSky is a plus).
• Reliable Transportation: Valid driver’s license, auto insurance, and access to a dependable vehicle.
