Job summary

Jet Health, Inc. Boise Home Health team is seeking a passionate RN Case Manager to provide personalized, in-home nursing care and coordinate patient plans.

You will build relationships with patients and families, work independently with strong team support, and deliver compassionate, patient-centered care.

Qualifications include an Idaho RN license (or multi-state), Home Health experience preferred, OASIS knowledge, and HCHB EMR proficiency; strong communication and time management are essential.

• Valid driver’s license and reliable transportation
