BrightStar Care of Chicago and La Grange is seeking an RN Clinical Lead for Home Care in downtown Chicago. The role requires strong critical care judgment and the ability to manage a mobile, client-facing schedule.

The ideal candidate will lead field nurses, coordinate care plans, and maintain high standards of clinical quality and safety. The position offers a salaried, Monday–Friday schedule with on-call as needed.

Qualifications

• Active, unrestricted RN license in Illinois (Registered Nurse)
• Experience in critical care settings with vents, trachs, G-tubes preferred
• Experience with geriatric care; pediatric experience a plus
• Ability to drive daily in downtown Chicago area and surroundings
• Strong communication and leadership skills
• Proficiency with electronic documentation systems
• Reliable vehicle and valid driver's license

Responsibilities

• Conduct comprehensive new patient assessments in clients' homes
• Perform scheduled and as-needed re-assessments to evaluate changes in patient condition
• Manage active caseloads, coordinate care plans, and document care continuity
• Serve as clinical lead and resource for field nurses and families
• Communicate patient status and plan changes with physicians and interdisciplinary teams
• Ensure care plans and documentation meet Joint Commission standards

Position: RN Clinical Lead- Home Care (Registered Nurse Required)

Shift: Salaried, Monday-Friday 8am-5pm

On-Call: As Business Needs Require

Experience Needed: Critical Care, Vent, Trach, G-Tube, Geriatrics (Pediatric Experience a Plus) (Home Care or Home Health Experience Preferred)

Location: Downtown Chicago (Must be willing to commute daily to downtown Chicago)

Pay: Experience Based $85,000 to $90,000

We are looking for an RN Clinical Leadwho knows home care - someone who is comfortable managing complex patients, supporting caregivers in the field, communicating with families and healthcare professionals, and maintaining the highest standards of clinical quality and safety. BrightStar Care of Chicago and La Grange is Joint Commission Accredited.

Key Responsibilities

• Conduct comprehensive new patient (start of care) assessments in clients' homes
• Perform scheduled and as-needed re-assessments to evaluate changes in patient condition and care needs
• Manage an active caseload, coordinating care plans, updating documentation, and ensuring continuity of care
• Serve as a clinical point of contact for field nurses, answering questions and providing guidance on complex cases
• Communicate with physicians, families, and interdisciplinary team members regarding patient status and care plan changes
• Ensure care plans and documentation meet regulatory and Joint Commission compliance standards

Qualifications

Required:

• Active, unrestricted RN license in the state of Illinois (Registered Nurse)
• Excellent Communication and Organization Skills
• Minimum 1 year of Critical care clinical experience (Vents,Trachs, G-Tubes, Infusion)
• Experience with Geriatrics
• Experience with Pediatrics a plus
• Reliable personal vehicle, valid driver's license, and current auto insurance
• Comfortable with extensive daily driving across the downtown Chicago area and surrounding communities
• Negative TB Test and Physical
• Current CPR Certification
• Strong assessment, critical thinking, and clinical decision-making skills
• Excellent communication skills - comfortable being a clinical resource for peers
• Proficiency with electronic documentation systems

Preferred:

• Minimum 1 year of ICU experience
• Home care, hospice, or case management experience
