The ideal candidate will hold an Illinois RN license, BSN, 3+ years of bedside nursing with at least 2 years in home health, and strong leadership and communication skills.

Responsibilities:

• Supervise and coordinate the activities of a team of home health nurses, therapists, and other healthcare professionals to ensure effective and efficient patient care delivery
• Conduct patient assessments, develop and implement care plans, and provide guidance and support to staff members
• Investigate and resolve patient complaints, ensure compliance with regulatory requirements, and maintain accurate records and reports
• Collaborate with physicians, social workers, and other healthcare professionals to ensure seamless communication and coordination of patient care
• Participate in on-call rotations, respond to emergencies, and provide guidance to staff members after hours
• Develop and implement quality improvement initiatives, monitor patient outcomes, and identify areas for improvement

Requirements:

• Current Illinois RN license and BSN degree in Nursing or related field
• At least 3 years of experience as a registered nurse within the last 5 years, and with a minimum of 2 years in home health care
• Strong leadership and communication skills, with the ability to work effectively in a fast-paced environment
• Excellent clinical knowledge and critical thinking skills, with the ability to analyze complex patient situations and develop effective care plans
• Ability to lift up to 25 pounds, travel to patient homes, and work in a dynamic environment
