Job Summary

The Registered Nurse in Home and Community Based Services provides and oversees nursing and personal care to patients in their homes, as prescribed by physicians and in compliance with laws, regulations, and agency policies. The RN coordinates care plans with healthcare professionals and ensures continuity of care by exchanging information with physicians, agency staff, patients, families, case managers, and community resources.

Specific Job Duties/Responsibilities

• Deliver high-quality clinical services within your scope of practice and infection control standards, following the care plan and collaborating with the care team from admission to discharge.
• Conduct clinical nursing assessments as required by federal, state, and payer requirements.
• Verify patient eligibility and medical necessity for services.
• Develop and update individualized care plans in collaboration with community providers.
• Adjust care plans as needed with physician approval to meet patient needs.

Experience Desired

• At least one year of RN experience preferred.
• Current RN licensure in the practicing state.
• Valid driver’s license, vehicle insurance, and access to reliable transportation.
• Current CPR certification.
