ComForCare Home Health Care - Northern CO in Colorado is seeking an experienced Registered Nurse Case Manager for Greeley and surrounding areas. You will oversee client caseloads, guide caregivers and build lasting relationships with clients and families, delivering compassionate in-home care.

Qualifications

• Active, unencumbered Colorado RN license.
• Graduate of an accredited school of nursing.
• Current BLS/CPR certification.
• Minimum 1 year of home care or home health nursing experience.
• Case management or caregiver supervision experience preferred.

Responsibilities

• Client intake and assessments: perform initial and ongoing in-home nursing assessments.
• Care plan development: create and update individualized plans of care.
• Caseload management: oversee a caseload and adjust services as needs change.
• Caregiver supervision and training: conduct supervisory visits and competency checks.
• Care coordination: serve as the clinical contact for families, physicians, hospitals and partners; make referrals as needed.
• Documentation and compliance: maintain client records per Colorado home care regulations and HIPAA.
• On-call support: assist with client and caregiver concerns.

What You’ll Do as an RN Case Manager

• Client intake and assessments: Complete initial and ongoing in-home nursing assessments of each client’s physical, cognitive, psychosocial and home safety needs
• Care plan development: Create and update individualized plans of care and make sure caregivers understand and follow them
• Caseload management: Oversee a caseload of home care clients, tracking changes in condition and adjusting services as needs change
• Caregiver supervision and training: Conduct supervisory visits and competency checks
• Care coordination: Serve as the main clinical contact for families, physicians, hospitals and community partners; make referrals when needed
• Documentation and compliance: Keep accurate, timely client records and follow Colorado home care regulations, infection control and HIPAA guidelines
• On-call support: Help resolve client and caregiver concerns quickly and professionally

Qualifications

• Active, unencumbered Registered Nurse (RN) license in Colorado
• At least 1 year of home care or home health nursing experience (required)
• Case management, care coordination or caregiver supervision experience (preferred)
• Graduate of an accredited school of nursing
• Current BLS/CPR certification
