Job Overview

We are seeking a compassionate and dedicated Registered Nurse (RN) to provide high-quality in-home care for patients in Brick, NJ (Ocean County). Key Responsibilities

• Deliver in-home nursing care by assessing, planning, implementing, and evaluating individualized care plans for patients
• Provide holistic and effective care, ensuring compliance with physician orders and nursing best practices
• Conduct thorough patient assessments, update care plans as necessary, and perform medication reconciliation
• Collaborate with physicians, interdisciplinary teams, and case managers to ensure coordinated patient care
• Serve as a clinical supervisor for home health aides, conducting evaluations and ensuring the highest quality of care delivery
• Educate patients and caregivers on treatment plans, medications, and overall health management
• Maintain accurate and detailed documentation to ensure compliance with regulatory and payor requirements
• Adhere to organizational policies, ethical standards, and safety protocols to uphold the highest standards of nursing practice

Qualifications

Required:

• Associate’s Degree in Nursing (ADN) or Diploma in Nursing
• Minimum of 6 months of RN experience (or 1 year of LPN experience); home care or hospice experience preferred
• Active New Jersey State Professional Registered Nurse (RN) License
• American Heart Association (AHA) Basic Life Support (BLS) Certification
• Proficiency in basic computer skills for documentation and care coordination

Preferred:

• Bachelor of Science in Nursing (BSN)
• Previous experience in home care or hospice nursing
