Job summary

Trinity Health PACE Alexandria is seeking an RN for home care to deliver skilled nursing in participants' homes, coordinating with an interdisciplinary team to support independence and well-being.

The role requires a nursing degree, Louisiana RN license, and at least one year of nursing experience, preferably in home care or geriatrics, with a flexible schedule including some weekends. Comprehensive benefits and ongoing professional development are provided.

Qualifications

• Graduate of an accredited School of Nursing; Associate or Bachelor’s degree required.
• Current Registered Nurse (RN) license in Louisiana.
• Minimum of one year nursing experience; home care/geriatric/PACE preferred.
• Valid driver’s license with reliable transportation and a clean driving record.
• Ability to work a flexible schedule, including occasional weekends.

Responsibilities

• Provide skilled nursing care to participants primarily in their homes, ensuring safety, comfort, and quality outcomes.
• Conduct comprehensive assessments and develop individualized care plans with the interdisciplinary team.
• Monitor participant health status and respond to changes in condition.
• Administer medications, treatments, and interventions as ordered.
• Educate participants and caregivers on disease management, medications, and care plans.
• Coordinate care with physicians, therapists, and other care team members.
• Document care in accordance with regulatory and organizational standards.
• Participate in interdisciplinary team meetings and contribute to care planning discussions.
• Ensure timely communication of participant needs, concerns, and changes in condition.

Position Purpose

The Registered Nurse (RN - Home Care) is responsible for delivering skilled nursing care to PACE participants in their homes, ensuring comprehensive, coordinated, and compassionate care. This role collaborates closely with the interdisciplinary team to assess, plan, and implement care that supports participants’ independence and overall well-being.
