Job summary

Americare Plus is hiring an Admissions RN to perform in-home assessments and develop personalized care plans. You will coordinate admissions paperwork and ensure compliance with agency policies, while communicating with families, caregivers, and physicians to ensure continuity of care.

The role requires compassion and clinical expertise during the transition to in-home care. Flexible scheduling accommodates part-time or full-time commitments, with competitive pay based on experience.

Qualifications

• Active RN license in Virginia.
• Minimum 1 year of nursing experience (home health or admissions experience preferred).
• Strong assessment and communication skills.
• Reliable transportation and a valid driver’s license.
