West State Street is seeking a Registered Nurse (RN) to deliver high-quality nursing care in a long-term care setting. You will perform assessments, develop care plans, administer medications, and collaborate with physicians, therapists, and family members to ensure optimal outcomes.

You will lead and mentor LPNs and CNAs, maintain documentation in PCC, and ensure compliance with HIPAA and safety guidelines. A valid RN license and BLS are required, with LTC experience preferred.

Qualifications

• Active RN license in the state where the facility is located.
• Experience in long-term care or geriatric nursing preferred.
• Proficient in documenting care and communicating with the care team.

Responsabilités

• Deliver high-quality nursing care under supervision to achieve positive patient outcomes.
• Assess patients, develop care plans, and coordinate with interdisciplinary team.
• Mentor LPNs and CNAs and ensure safe discharge planning.
