Job summary

Avamere Three Fountains in Medford, OR is seeking a Registered Nurse (RN) for full-time day shift (6am–6pm). You will assess residents, develop care plans, administer medications, monitor health, and coordinate care with the interdisciplinary team, while supervising LPNs and CNAs.

Requirements: RN degree from an accredited program, active OR RN license (BSN preferred), CPR/BLS current, and EMR experience in long-term care.

Qualifications

• Must have an RN degree from an accredited college or university.
• Active RN license in Oregon; BSN preferred.
• Must maintain CPR/BLS certification.
• Prefer experience in long-term care or geriatric settings.
• Experience with electronic medical records (EMR).

Responsibilities

• Assess residents and develop individualized care plans with the interdisciplinary team.
• Administer medications and monitor patient responses.
• Monitor vital signs and document health status.
• Supervise LPNs and CNAs; provide staff orientation.
• Communicate with residents, families, and health professionals.
• Participate in quality improvement initiatives.
