Job summary

Yale NewHaven Health is seeking a Registered Nurse for the Westerly area to provide individualized, goal-directed nursing care under the nurse manager's direction. You will apply the nursing process and primary nursing principles per department policies.

Responsibilities include ongoing patient assessment, planning and implementing care, evaluating outcomes, and ensuring compliance with hospital standards. BLS is required; ACLS optional; PALS within one year of hire.

Qualifications

  • Must be a graduate of an accredited School of Nursing with RN licensure.
  • Rhode Island RN license and BLS required; ACLS optional; PALS within one year of hire.
  • Medical-surgical nursing, pediatrics and telemetry experience preferred.

Responsibilities

  • Assess patient's physical and psychological condition continually and document data.
  • Develop and communicate an individualized plan of care with patient/family involvement.
  • Implement the medical plan and nursing interventions per standards of practice.
  • Evaluate the plan's effectiveness and adjust nursing activities to support goals.
  • Adhere to Westerly Hospital policies and exposure control plans.
  • Perform other duties to ensure quality patient care and department operations.
  • Act as a clinical resource and role model for staff and self-development.
  • Demonstrate high level of respect, dignity, compassion and professionalism to all customers.
  • Promote collaboration and teamwork across the care team.
  • Ensure punctuality and adherence to hospital attendance policies.
  • Maintain accurate documentation and patient data handling.

Under the direction of the nurse manager, manages individualized, goal-directed nursing care through the use of the nursing process and the principles of primary nursing, in accordance with departmental and hospital policies and procedures.

Responsibilities
  • 1.Assesses the patient's physical and psychological condition on an ongoing basis throughout the continuum of care. Documents appropriately the information obtained from the ongoing assessment. Shares significant assessment data with the appropriate discipline.
  • 2.Using the four tenets of Patient Family Centered Partnership, develops, communicates and documents a plan of care reflecting individualized patient problems and interventions to achieve the desired outcomes. Incorporates feedback from patient, family and all disciplines in developing the plan of care.
  • 3.Uses critical thinking skills and judgment necessary to implement the medical plan of care, nursing interventions, and procedures as necessary for the care of the patient. Demonstrates basic clinical and technical skills according to established standards of practice. Anticipates potential changes in the patient's status and assesses the patient accordingly.
  • 4.Evaluates the effectiveness and patient's response to the plan of care. Identifies appropriate nursing activities which contribute to the patient's progress towards achieving both short and long term goals.
  • 5.Adheres to The Westerly Hospital policies and procedures
  • 6.Adheres to the hospitals exposure control plans/blood- borne and airborne pathogens
  • 7.Performs other duties as assigned or directed to ensure the smooth operation of the department to provide quality patient care.

Qualifications

EDUCATION

Graduate of an accredited School of Nursing. BSN preferred.

EXPERIENCE

Medical-surgical nursing, pediatrics, telemetry experience preferred.

LICENSURE

Valid State of Rhode Island Registered Nurse License, BLS required. ACLS (Optional). PALS within one year of hire

SPECIAL SKILLS

Demonstrated proficiency in acute-care nursing, knowledge, and skills. Ability to communicate effectively and document information accurately. Ability to deal efficiently and effectively with a wide variety of individuals. Basic computer knowledge is preferred. Strong ability to communicate and interact professionally in busy environment.

PHYSICAL DEMAND

Bending, carrying items up to 50 pounds, sitting/on phone for longer periods of time. Must be able to stand or walk for long periods of time.
