Deep Creek Health & Rehabilitation in Chesapeake, Virginia is seeking Registered Nurses (RNs) to provide compassionate, resident-centered care in a skilled nursing and long-term care setting.

You will administer medications, plan individualized care, educate residents and families, and collaborate with a multidisciplinary team to ensure quality outcomes. This full-time opportunity offers competitive hourly pay, shift differentials, benefits, and growth opportunities.

Qualifications

• Active state RN license is required.
• CPR/BLS certification is required.
• Experience in Long Term Care or Skilled Nursing preferred.
• Excellent technical, assessment and documentation skills.
• Strong analytical and problem-solving skills.
• Ability to lift 25+ pounds.
• Ability to stand, bend, and walk for extended time.
• Team player and strong communicator.

Responsibilities

• Provide nursing care to meet resident needs per standards of practice, physician orders, center policies and guidelines.
• Plan nursing interventions for each resident based on individual needs and monitor implementation.
• Evaluate effectiveness of care interventions and develop alternatives.
• Conduct family and resident education.
• Administer medications and treatments timely and according to center policy.
• Transcribe physician orders and chart progress notes accurately.
• Identify and report changes in condition to supervisor, physician and family.
• Follow skin management protocols and identify skin changes.
• Order and report diagnostic tests timely.
• Coordinate rehabilitation program with therapists.
• Ensure CNA documentation and assignments are completed on time.
• Perform additional nursing care responsibilities as required by State Nurse Practice Act.
