Job summary

Bluegrass Care Navigators seeks a Flex/After-Hours Hospice RN to deliver compassionate, patient-centered care in a variety of home settings, including private homes and skilled facilities. This hybrid role supports care outside regular hours and offers flexible dayshift coverage based on team needs.

The RN must hold a current Kentucky nursing license, CPR certification, and be capable of autonomous work within an interdisciplinary team. Prior hospice or home-health experience is valued.

Qualifications

• Graduate of an accredited school of registered nursing.
• Currently licensed as a Registered Nurse through the Kentucky Board of Nursing or COMPACT state.
• One year of general nursing experience preferred; new grads considered.
• CPR certification required.
• Excellent observation and documentation skills.
• Must be able to work autonomously within an interdisciplinary team.

Job description

Schedule: 3 13-hour shifts every 2 weeks

One weekday 8:00am-9:00pm every other week & every other Saturday & Sunday 8:00am-9:00pm

Note: Weekday is flexible and will be determined in scheduling with the supervisor

Incentive: $4.00 per hour on-call shift differential pay for shifts after 5:00pm weeknights and all day for weekend shifts, in addition to base pay

Position Summary:

The Flex/After-Hours Hospice RN delivers compassionate, patient-centered care for individuals receiving comprehensive health care services in a variety of home care settings, including private residences, skilled nursing facilities, personal care homes, or wherever the patient calls home. This hybrid role supports continuity of care outside of regular business hours and also provides flexible dayshift coverage based on the evolving needs of the hospice team. These needs may include responding to urgent patient needs, managing symptoms, supporting new admissions, and assisting with ongoing case management. A Registered Nurse in this position will have strong communication skills, the competence to work autonomously, professionalism to work within an interdisciplinary team, and the ability to provide care with empathy. Requirements:

• A graduate of an accredited school of registered nursing.
• Currently licensed as a Registered Nurse through the Kentucky Board of Nursing or participating COMPACT state Board of Nursing.
• One year of general nursing experience is preferred. New nursing graduates will be considered.
• Skilled in nursing practice and able to maintain professionalism in moments of emotional stress.
• Provide care in a compassionate nature while serving a diverse population respectfully and professionally.
• Complies with accepted professional and ethical standards and principles.
• Understands and supports the hospice philosophy and principles of death and dying.
• Possesses and maintains current CPR certification.
• Have satisfactory references from nursing schools, previous (or current) employers and/or professional peers.
• Have excellent observation, good nursing judgment and communication skills. Ability to provide accurate and timely documentation.
• Self-directed to work with minimal supervision and within an interdisciplinary team
• Flexible and cooperative in fulfilling role obligation to meet the needs of patients and BCN.
• Team members will be expected to comply with Bluegrass Care Navigators (BCN) Employee Health, infectious disease, and vaccination policies.
• Must be a licensed driver with an automobile in good working order, that is insured in accordance with state and/or organization requirements, with personal auto liability insurance of $100,000/$300,000 in bodily injury coverage.
