Job summary

Bluegrass Care Navigators is seeking a Flex/After-Hours Hospice RN to deliver compassionate, patient-centered care in home and facility settings across Kentucky. This hybrid role supports continuity of care outside regular hours and works with an interdisciplinary team to manage symptoms and support new admissions.

Applicants should be a graduate of an accredited RN program, licensed in Kentucky or a Compact State, CPR certified, and possess a valid driver's license.

Qualifications

• Graduate of an accredited school of registered nursing.
• Currently licensed as a Registered Nurse through the Kentucky Board of Nursing or a participating Compact State Board of Nursing.
• Minimum of one (1) year of general nursing experience preferred; new graduate nurses will be considered.
• Demonstrates strong clinical nursing skills and the ability to maintain professionalism during emotionally challenging situations.
• Provides compassionate, patient-centered care while serving diverse populations with respect and cultural sensitivity.

Responsibilities

• Deliver compassionate, patient-centered home care in a variety of settings, including private residences, skilled nursing facilities, personal care homes, or wherever the patient calls home.
• Support continuity of care outside of regular business hours and provide flexible day-shift coverage based on evolving needs.
• Respond to urgent patient needs, manage symptoms, support new admissions, and assist with ongoing case management.
• Collaborate with an interdisciplinary team to provide care with empathy and professionalism.
