Job summary

LiveWell Homecare Agency, LLC is seeking an RN to provide direct nursing care and implement patient care plans at UAB St. Vincent's Birmingham. The role involves coordinating with the care team, monitoring patient conditions, and communicating updates to physicians and staff.

Responsibilities include ensuring orders are carried out and assisting with discharge planning as part of a collaborative healthcare team. Flexible full-time day/night or weekend shifts available.

Qualifications

• Current or eligible RN license by the Alabama Board of Nursing.
• BLS certification within 30 days of hire from AHA or Red Cross.
• Diploma from an accredited nursing school and licensure at hire.
• Shifts available: full-time days and nights, or weekend days.
• Work location: UAB St. Vincent's Birmingham.

Responsibilities

• Provide direct nursing care following organizational policies and protocols.
• Implement and monitor patient care plans, assess patient conditions, and communicate updates.
• Coordinate multidisciplinary care and ensure physician and nursing orders are carried out.
• Assist with discharge planning in collaboration with the healthcare team.
