Job summary

Great Livin' LLC seeks a full-time registered nurse to provide nursing services to adults with developmental disabilities in NM. Duties include assessments, care plans, and coordination with HCC/IDT teams within waiver programs.

The RN will ensure documentation accuracy, oversee CMA staff, and liaise with medical providers per NM regulations and Medicaid standards. Strong English, integrity, and independent work ability are essential.

Qualifications

  • Must be able to speak, read and write in English.
  • Proficient in detailed documentation and health services software.
  • Knowledge of NM ICF-IDD standards and nursing regulations.
  • Ability to prioritize tasks and work independently.
  • Strong integrity and stable health and emotional state.
  • Experience with coordinating care across teams and providers.

Responsibilities

  • Perform initial nursing assessments and care planning for individuals in the waiver program.
  • Create and submit required care plans, MERPs, and CARMPs per state standards.
  • Provide nursing oversight and education to CMA staff and supervisors.
  • Review and communicate medication changes, appointments, and health tracking.
  • Document care in Therap and coordinate with HCC and IDT teams.
  • Attend health-related meetings and audits as required.
  • Coordinate with medical providers and other agencies for health concerns.
  • Oversee CMA program training and CMA supervision as needed.
