Job summary

Developmental-Disability-Center-of Nebraska (DDCN) in Omaha is seeking a Licensed Practical Nurse experienced with Adult DD Waiver services to join our full-time team. You will provide direct nursing care, monitor health status, coordinate with physicians, and support individuals to maintain independence within the community.

Qualifications

Experience with adult habilitation and waiver programs.

Responsibilities

Communicates health/nursing needs and recommendations to staff, other health providers and families.
Coordinates hospital admissions, discharges, and follow-up as needed.
Monitors health status and ensures nursing services align with the Service Plan.
