Responsibilities

• Administer medications including oral, topical, IV, injectable.
• Perform wound care/prevention and manage pain.
• Conduct health assessments and track changes.
• Create, update & execute individualized care plans.
• Collaborate with physicians, therapists, and social workers.
• Educate families on health status and care expectations.
• Document health status, care, and interactions.
• Oversee and delegate to LPNs and CNAs.
