The Hospitals of Providence - Sierra Campus is seeking an experienced registered nurse to coordinate patient care across the continuum. You will develop customized care plans with primary physicians to meet individual patient needs and preferences.

As part of a collaborative team, you will ensure timely tests and treatments, advocate for patients, and balance clinical and financial factors to deliver efficient care. Full-time, day shifts are available.

Qualifications

• Academic nursing degree required; two years of nursing experience preferred.
• Current license or certification to practice nursing in state of employment (AHA BLS).
• Bachelor's or master's degree preferred for advanced roles.

Responsabilités

Job Summary

Full Time, Days

Coordinates the care needs of assigned patients and develops comprehensive plans to manage care delivery across the patient care continuum. Partners with patients and their primary physicians to develop customized care plans based on their individual needs and preferences.

Responsibilities

Collaborates with physicians, nurses, allied health professionals, social work, and others to ensure appropriate tests and treatments are delivered in a timely fashion. Advocates for the patient. Balances care needs and financial considerations to ensure efficient and effective treatments are achieved.

Qualifications

Education
Required: Academic degree in nursing.

Preferred: Bachelor's or master's degree.

Experience
Required: 2 years nursing experience.

Certifications
Required: Currently licensed, certified, or registered to practice profession as required by law, regulation in state of practice or policy. AHA BLS.
