The Hospitals of Providence - Sierra Campus is seeking a nurse to coordinate care for assigned patients, developing comprehensive plans for delivery across the patient care continuum. You will partner with patients and primary physicians to tailor plans to individual needs and preferences.

The role emphasizes collaboration with medical teams, timely tests and treatments, patient advocacy, and balancing care with financial considerations to optimize outcomes.

Qualifications

• EducationRequired: Academic degree in nursing. Preferred: Bachelor's or master's degree.
• ExperienceRequired: 2 years nursing experience.
• CertificationsRequired: Currently licensed, certified, or registered to practice profession as required by law, regulation in state of practice or policy. AHA BLS.
