Job summary

Qualifications

Responsibilities

• Assess patient needs through observation, interview, records review and data.
• Develop and implement patient care plans with interdisciplinary team.
• Perform therapeutic/diagnostic interventions (IVs, wound care, meds, catheterization, specimen collection).
• Maintain confidential patient records with accurate documentation and IT communication.
• Promote patient safety and advocate for patient rights.
• Provide patient/family education based on learning readiness and needs.
