Job summary

BrightStar Care of Concord, Lexington & Woburn is seeking an experienced Registered Nurse Clinical Manager to lead our home care nursing team. This role focuses on Plan of Care management, clinical compliance, and staff supervision in a fast-paced home health setting.

As RN Manager you will develop care plans, ensure regulatory alignment, train staff, and drive quality patient outcomes across our local agencies.

Qualifications

• Active RN license in the operating state.
• 2–3 years of home health, hospice, or skilled nursing experience.
• Proficient in IV therapy and patient care planning.
• Strong leadership and staff supervision abilities.
• Ability to develop and oversee Plan of Care.

Responsibilities

• Develop and implement patient care plans with physicians and the team.
• Supervise the home care nursing staff and coordinate scheduling.
• Ensure compliance with state and federal home health regulations.

Registered Nurse (RN) Job Responsibilities:

• Develop and implement patient care plans in collaboration with physicians and other healthcare providers.
• Conduct in-home assessments and ensure that patient care meets regulatory and company standards.
• Oversee compliance with state and federal home health regulations, including documentation and reporting requirements.
• Supervise and manage the home care nursing team, including hiring, training, and evaluating staff performance.
• Provide guidance and support to nurses and caregivers to ensure the highest level of patient care.
• Manage scheduling and ensure adequate staff coverage for all home care cases.

Registered Nurse (RN) Qualification and Requirements:

• Active Registered Nurse RN License – Must hold a valid and unrestricted Registered Nurse (RN) license in the state of operation.
• Clinical Experience – At least 2-3 years of experience in home health, hospice, or skilled nursing.
• IV Infusion Therapy Expertise – Proficient in administering and overseeing IV infusions, including medications, hydration, and TPN (total parenteral nutrition)
• Sales & Business Development Mindset – Ability to attract and retain clients and collaborate with referral partners
• CPR Certification
• Leadership & Management – Strong ability to supervise, train, and mentor nurses, aides, and caregivers.
• Care Plan Development – Skilled in creating and overseeing patient care plans, ensuring high-quality outcomes.
• Quality Assurance & Risk Management – Experience in auditing, incident reporting, and maintaining compliance with policies.
• Problem-Solving & Decision-Making – Ability to handle staffing issues, patient concerns, and operational challenges quickly and efficiently.
• Reliable Car, Driver's License and Auto Insurance (Required)
• Flexibility & Availability – Comfortable with on-call responsibilities and responding to emergencies

Nice to Haves:

• Home Health or Private Duty Nursing Experience – Familiarity with BrightStar Care’s business model is a plus

As the Home Care Registered Nurse (RN) Manager, you will play a crucial role in managing the clinical operations of our home care services. This Registered Nurse Clinical Manager (RN) leader will own the overall responsibility for quality, adequacy and reliability of our home care services program – Plan of Care management, staff supervision and clinical compliance.
