BrightStar Care of Concord, Lexington & Woburn seeks an experienced Home Health RN Manager to lead clinical operations for our home care services. You will own Plan of Care management, supervise staff, and ensure clinical compliance.

As a Registered Nurse, you will ensure high-quality patient care, guide nurses and caregivers, and maintain regulatory standards while providing flexible scheduling and opportunities for advancement.

Qualifications

• Active RN license in the state of operation.
• 2–3 years of home health, hospice, or skilled nursing experience.
• Proficient in IV infusion therapy.
• Sales and business development mindset to attract and retain clients/referral partners.
• CPR certification.
• Strong leadership and staff supervision abilities.
• Care plan development and oversight experience.
• Quality assurance/risk management experience.
• Problem-solving and quick decision-making in staffing and operations.
• Reliable car and valid driver's license.

Responsabilités

• Develop and implement patient care plans in collaboration with physicians and other healthcare providers.
• Conduct in-home assessments and ensure patient care meets regulatory and company standards.
• Oversee compliance with state and federal home health regulations, including documentation and reporting requirements.
• Supervise and manage the home care nursing team, including hiring, training, and evaluating staff performance.
• Provide guidance and support to nurses and caregivers to ensure the highest level of patient care.
• Manage scheduling and ensure adequate staff coverage for all home care cases.

Registered Nurse (RN) Job Responsibilities

Registered Nurse (RN) Qualification and Requirements

Must Haves:

• Active Registered Nurse RN License – Must hold a valid and unrestricted Registered Nurse (RN) license in the state of operation.
• Clinical Experience – At least 2-3 years of experience in home health, hospice, or skilled nursing.
• IV Infusion Therapy Expertise – Proficient in administering and overseeing IV infusions, including medications, hydration, and TPN (total parenteral nutrition)
• Sales & Business Development Mindset – Ability to attract and retain clients and collaborate with referral partners
• CPR Certification
• Leadership & Management – Strong ability to supervise, train, and mentor nurses, aides, and caregivers.
• Care Plan Development – Skilled in creating and overseeing patient care plans, ensuring high-quality outcomes.
• Quality Assurance & Risk Management – Experience in auditing, incident reporting, and maintaining compliance with policies.
• Problem-Solving & Decision-Making – Ability to handle staffing issues, patient concerns, and operational challenges quickly and efficiently.
• Reliable Car, Driver's License and Auto Insurance (Required)
• Flexibility & Availability – Comfortable with on-call responsibilities and responding to emergencies

Nice to Haves

• Home Health or Private Duty Nursing Experience – Familiarity with BrightStar Care’s business model is a plus
