Job summary

Tower Health is seeking an ambulatory nurse to provide competent clinical care across the life-span and coordinate care across disciplines. The role emphasizes telephonic triage, patient education, and direct care interventions to enhance outcomes.

You'll facilitate transitions across care settings and collaborate with a multidisciplinary team within Tower Health's system to improve quality and reduce unnecessary utilization and costs across patient populations.

Qualifications

• Bachelor of Science in Nursing or equivalent certification or RN to BSN/MSN enrollment with 12 credits completed in 12 months
• Completes and maintains all required competencies and trainings for the role
• Active PA Registered Nurse License and BLS Certification required or in process

Job Summary

• Provides competent clinical nursing practice across the life-span, facilitates transitions across care settings, and coordinates care delivery with other disciplines for improving patient care quality outcomes as well as decreasing unnecessary utilization and associated costs across patient populations and health care settings.
• Daily ambulatory nursing practice performance primarily is focused on telephonic patient triage, patient education, direct care interventions, collaborative leadership and care coordination.
