Job summary

Grand Itasca Clinic is seeking a Registered Nurse to join the Clinic Care Team. The role focuses on triaging patients by phone, coordinating care with providers, and supporting pre-visit planning and post-visit follow-up to ensure efficient, high-quality care both in-person and virtually.

The RN will work with clinic providers and a multidisciplinary team, performing pre-visit planning, patient education, and care coordination across the patient journey while following Minnesota RN practice

Qualifications

• Current MN RN license to practice.
• Knowledge of Minnesota Nurse Practice Act and ambulatory care.
• Current BLS certification (or willing to obtain within 2 months).
• Experience in family medicine clinic workflows and patient education.

Responsibilities

• Triages patients over the phone and directs care with best clinical judgement.
• Collaborates with physicians and nursing staff to coordinate patient care.
• Performs pre-visit planning to maximize in-clinic appointment time.
• Completes Transitional Care Management (TCM) calls for discharged patients.

Job Summary

This role reports to one of the two RN Clinic Managers and is part of the clinical care teams responsible for triaging Grand Itasca patients over the phone to direct appropriate care while supporting the clinic providers through pre-visit planning and patient care coordination before, during, and after clinic appointments, both in-person and virtually. Clinical Care Team RN is assigned to group or provider panels and be an ongoing resource between patients and providers for patient care management. This role assists clinical care teams with any patient care requiring an RN.

Job Duties

1. Follows the RN process for nursing assessment, diagnosis, planning, implementation of care, and evaluation of patient progress to function in a full scope RN role to support providers in the clinic.
2. Acts as a resource to LPN and CMA staff in assigned unit in accordance with the Minnesota Nurse Practice Act (NPA).
3. Cover provider in-baskets on off days.
   
4. Works collaboratively with physicians/providers and nursing personnel to triage patients over the phone, directing them appropriately with best clinical judgement.
   
5. Works collaboratively with clinic personnel/departments and hospital personnel/departments to support smooth patient transitions.
   
6. Provides evidence-based patient and staff education relating to clinical care.
   
7. Completes Transitional Care Management (TCM) calls for hospital and emergency department discharged patients, ensuring patients are contacted to reduce risk of readmission and obtain TCM payment.
8. Completes RN only assignments, i.e. blood pressure checks, EKGs, GCM placements.
   
9. Follows-up with patients after visits and assists to coordinate follow-up appointments with primary or specialty care.
   
10. Performs pre-visit planning work to obtain reason for visit, pharmacy, chief complaint, etc. prior to visit to ensure in-clinic appointment time is well utilized with provider.
    
11. Completes Ambulatory Care Composite outreach.
12. Triages patients over the phone and scrubs MyChart messages for triage opportunities.
    
13. Completes and routes patient medication refill requests through the EHR and Right Fax when Clinic Refill RN needs support.
14. Completes medication review prior to appointments and for nursing home patients.
15. Knows the key staff available to help patients with financial and emotional support; arranges for patients to meet with these resources people, if needed.
    
16. Hours and days of work may vary to manage organization’s needs in accordance to the RN contract.
    
17. Manages scheduled medication contracts for unit patients.
    
18. Other duties in support of unit patient’s plan of care and consistent with full scope RN practice in accordance with Minnesota Nurse Practice Act.

1. Has a current RN license to practice nursing in the state of Minnesota.
2. Demonstrated knowledge of the Minnesota Nurse Practice Act and the Scope of Ambulatory Care Nursing.
3. Demonstrates clinical competence in providing direct patient care and patient and family education as well as assisting the physician in the delivery of patient care.
4. Has current BLS certification (or is willing to obtain within two months of employment.)
5. Demonstrated ability to manage the complex needs of a family medicine practice.
6. Demonstrated competence in managing patients using Patient Care Protocols.
7. Ability to manage change in a positive patient centered manner.
8. Clinic experience preferred.
9. Demonstrated ability to follow and adhere to Grand Itasca Clinic and Hospital policies, vision and mission.
10. Maintains reliable and consistent attendance.
