Job summary

Signature HealthCARE in Monteagle, TN seeks an experienced Registered Nurse to provide direct patient care, lead nursing assistants, and coordinate care plans in a skilled nursing setting. You will administer medications, monitor resident conditions, and document care in the EHR while ensuring safety and dignity for residents.

The role requires an active RN license in Tennessee, with preference for long-term care experience and supervisory exposure.

Qualifications

• Current, active RN license in the state of practice.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.
