Job summary

Signature HealthCARE is seeking a registered nurse for our Monteagle Rehab & Wellness Center. You will provide direct care, supervise staff, and ensure safety and quality for residents in a 150-bed facility.

Applicants should hold an active RN license in Tennessee (BLS/CPR preferred), with at least one year of clinical nursing experience and supervisory exposure. Strong documentation and EHR skills are essential.

Qualifications

• Active RN license in the state of practice.
• CPR/BLS preferred (American Heart Association or American Red Cross).
• Minimum one year of clinical nursing experience; long-term care preferred.
• Experience in a supervisory or charge nurse role preferred.
• Strong clinical observation, medication administration and documentation skills.
• Familiar with Federal/State long-term care regulations.
• Strong communication and ability to work independently.

Responsibilities

• Provide direct nursing care to residents including meds and wound care.
• Assist residents with ADLs, ensuring comfort, dignity, and safety.
• Supervise nursing assistants and medication aides per policy.
• Monitor residents and report changes to physician/party of record.
• Develop and review individualized care plans with the team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers and discharges.
• Participate in QA activities and regulatory surveys.
• Perform other duties as assigned by DON or designee.
