Position Summary

We are looking for an experienced registered nurse who initiates and coordinates the hospice care plan, supervises CHHAs and LVNs, and provides skilled nursing hospice and palliative care services to patients at their residences. The RN will work cooperatively with the attending physician, Hospice Medical Director, and other members of the Interdisciplinary Hospice Team. Responsibilities include reviewing and updating the care plan, performing prescribed medical treatments (including pain management and symptom control), conducting assessments, and providing education and supportive care to patients and caregivers. Must be comfortable using computers and electronic medical records.

Qualifications

• Graduate of an accredited school of nursing.
• Current California RN license. BSN preferred. Certification in Hospice and Palliative Nursing encouraged.
• Minimum of one-year experience as a professional nurse within the last three years or a Baccalaureate in Nursing from an NLN-accredited program.
• Valid driver's license, auto insurance, reliable and well-maintained transportation.
• Spanish speaking is a plus but not required.
