Full Time Hospice Registered Nurse Case Manager (RNCM)

Must be able to commute to the San Gabriel Valley areas (East LA/Pasadena/West Covina/Pomona)

Hourly: $50.00 + Mileage Reimbursement

POSITION SUMMARY

We are seeking an experienced registered nurse to initiate and coordinate hospice care plans, supervise CHHAs and LVNs, and provide skilled nursing hospice and palliative care services at patients' residences. The RN will collaborate with the attending physician, Hospice Medical Director, and other members of the Interdisciplinary Hospice Team. Responsibilities include reviewing and updating care plans, performing prescribed medical treatments (including pain management and symptom control), conducting assessments, and providing education and supportive care to patients and caregivers.

Must be proficient with computers and comfortable using electronic medical records.

QUALIFICATIONS

• Graduate of an accredited school of nursing
• Current California RN license; BSN preferred. Certification in Hospice and Palliative Nursing encouraged.
• Minimum of one year of professional nursing experience within the last three years or a Baccalaureate in Nursing from an NLN-accredited program.
• Valid driver's license, auto insurance, reliable and well-maintained transportation
• Spanish speaking skills are a plus but not required.
