Alternate Solutions Health Network is seeking a Registered Nurse Case Manager for hospice care in the Akron area. The role requires leading patient assessments, care planning, and coordinating interdisciplinary teams to deliver comprehensive, cost-effective, and billable care.

The RN will perform ongoing skilled visits, update orders, and communicate with physicians, patients, and families to ensure high quality end-of-life care.

Qualifications

  • Registered Nurse license required in state of employment.
  • Hospice experience preferred.

Responsibilities

  • Oversees the overall care of the patient from beginning to end of life.
  • Performs initial, comprehensive, and ongoing skilled nurse visits.
  • Updates doctor orders and notifies physician of significant changes.
  • Provides end-of-life care in patient homes as ordered by the physician.
  • Supervises care provided by nurse aides and LPNs.
  • Documents treatment, assessments, and patient education per protocol.

The RNCM provides case management, clinical care and is responsible for coordination all disciplines involved in providing quality, cost-effective and billable care through direct care and supervision of care under the direction of the physician. ASHN adopts the RN scope of practice as outlined by the state for its licensed RN personnel.KEY RESPONSIBILITIESOversees the overall care of the patient from beginning to end of lifeDoes initial, comprehensive, and ongoing skilled nurse visits for patient and family as determined by plan of careUpdates all doctor orders in patient chart accordinglyNotifies doctor of any significant changes of the patient’s conditionManages interdisciplinary care as applicableProvides end of life care in a patient’s home as ordered by the attending physicianObserves and monitors patient conditionsIndividualizes plan of care and adjusts as needs changeAdministers medication as prescribed by the physicianPromotes continuity of care with appropriate admissions, transfers and dischargesCounsels’ patient and family on the disease/progressionIncorporates patient and family in development of plan of careMaintain effective communication with patients, families, physicians and co-workers. Ability to remain calm, have patience and be accommodating. Compassionate and caring while working with patients and families.Oversees and supervises total care of patient provided by nurse aides and LPNsEvaluates the treatments and medical condition of the patient according to the plan of careDetermines if the level of care being provided meets the patients’ needsCommunicates with the LPNs and nurse aides on supervisory visit resultsCompletes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelinesDocuments all aspects of treatment, assessments, and patient educationCommunicates with scheduler any changes outside the normalParticipates in all on-call requirements and case conferencesAttends in-service trainings and mandatory agency meetingsOther duties as assignedQUALIFICATIONSOne year of experience as a RN in an acute care or home care setting is required.Hospice experience is preferred.Maintains active RN license in state of employmentUnderstands and practices agency policies and proceduresMaintains a professional appearance as a representative of the companyAbility to remain calm, have patience and be accommodating. Compassionate and caring while working with patients and families.Knowledgeable on nursing best practices. Ability to make appropriate nursing judgments.
