Summa Home Health and Hospice LLC in Akron, OH seeks a dedicated Registered Nurse Case Manager (RNCM) to lead patient assessments, plan of care, and coordination of interdisciplinary care in a home hospice setting.
You will perform skilled nurse visits, update orders, educate families, and ensure smooth transitions of care with physician direction. The role requires compassionate leadership and the ability to work calmly under pressure.

The Registered Nurse Case Manager (RNCM) - Hospice assumes ultimate accountability and leadership for the assessment of the patient condition and plan of care. The RNCM provides case management, clinical care and is responsible for coordination all disciplines involved in providing quality, cost-effective and billable care through direct care and supervision of care under the direction of the physician. ASHN adopts the RN scope of practice as outlined by the state for its licensed RN personnel.
