Job summary

Bristol Hospice in Clackamas, Oregon, is seeking a dedicated RN Case Manager for a full-time day shift. You will provide compassionate, one-on-one hospice care, supporting patients and families through a meaningful journey with dignity and comfort.

Responsibilities include care planning, comprehensive assessments, medication administration, family education, and close collaboration with an interdisciplinary team to ensure high-quality end-of-life care.

Qualifications

• Valid RN license in good standing in the work state.
• Current CPR certification and reliable transportation.
• Strong communication and compassionate patient care focus.

Responsibilities

• Develop and implement individualized care plans based on medical history.
• Deliver one-on-one support to patients and families, ensuring comfort and dignity.
• Assess physical, emotional, and spiritual needs throughout the care journey.
• Monitor vital signs and document changes accurately.
• Administer physician-ordered medications and treatments with empathy.
• Educate patients and families on care techniques and symptom management.
• Communicate changes in condition promptly and coordinate with the team.
• Maintain up-to-date clinical notes reflecting progress and care quality.
• Collaborate with interdisciplinary teams for comprehensive care.
• Contribute to additional duties as part of a mission-driven team.

• Hourly Salary Range: $50.00 – $53.00

• Schedule: Monday – Friday, 8:00am – 5:00pm

• Deliver attentive, one‑on‑one support to patients and their families, ensuring comfort, dignity, and understanding.
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey.
• Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure.
• Administer physician‑ordered medications and treatments with skill and empathy.
• Educate patients and their families on care techniques and symptom management.
• Track and communicate changes in patient condition, ensuring timely and effective responses.
• Keep detailed and up‑to‑date clinical notes to reflect patient progress and quality of care provided.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
• Contribute to other supportive duties as part of a compassionate, mission‑driven team.
• Additional duties as assigned.

Qualifications and Experience

• A heart for hospice care and a commitment to exceptional service.
• A valid Registered Nurse license in good standing in the state where you will work.
• A valid driver’s license and reliable transportation.
• Current CPR certification.
• Strong observational and communication skills—both written and verbal.
• A flexible, team‑oriented attitude and willingness to support additional duties as needed.
