Job summary

HCA Florida Lake Monroe Hospital in Sanford, FL is seeking an RN Case Manager to support patients and families through their care journey. You will coordinate services, connect resources, and develop care plans reflecting each patient’s needs, partnering with physicians, nurses, and care teams.

The role emphasizes safe transitions, clear communication, and high-quality, patient-centered care. A relevant nursing degree and acute hospital experience are required.

Qualifications

• APR N or RN required.
• Associate Degree or RN Diploma required.
• Acute hospital experience required.

Responsibilities

• Guide patients and families through program orientation, discharge criteria, and patient rights.
• Coordinate education for patients, families, and caregivers to support treatment planning.
• Develop and implement individualized treatment plans reflecting patient strengths and goals.
• Complete psychosocial assessments and discharge planning evaluations and document findings.
• Collaborate with Rehab Program Director and interdisciplinary team to ensure seamless care.
• Promote compassionate, respectful, and excellent patient interactions.

Job Summary And Qualifications

As a Case Manager, your role will be to support patients and families through every step of their care journey. You will coordinate services, connect resources, and develop care plans that reflect each patient’s unique needs. By partnering with physicians, nurses, and department leaders, you will help ensure safe transitions, clear communication, and consistent quality across the continuum of care.

Your responsibilities will include:

• Guiding patients and families through program orientation, explaining the rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights


• Coordinating education for patients, families, and caregivers to encourage participation in treatment planning, goal discussions, and family conferences


• Developing and implementing individualized treatment plans that reflect the patient’s strengths, needs, and personal recovery goals


• Completing psychosocial assessments and discharge planning evaluations, clearly documenting findings and communicating needs across the care team


• Collaborating closely with the Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless, patient-centered care


• Promoting HCA Healthcare’s values of compassion, respect, and excellence through every patient and family interaction

What qualifications you will need:

• (APRN) Advanced Practice Registered Nurse, or (RN) Registered Nurse


• Associate Degree, or Registered Nurse Diploma


• Acute Hospital experience Required
