Responsibilities

As the RN Case Manager, you will be responsible for the delivery of the Plan of Care for each hospice patient, including:

• Conducting initial nursing evaluations within 48 hours of referral to determine eligibility for hospice services.
• Assessing and re-assessing the patient's physical, psychosocial, and environmental needs at least every 15 days.
• Initiating and coordinating the plan of care, maintaining accurate clinical notes.
• Documenting problems, goals, interventions, and patient/family responses.
• Collaborating with the patient, family, attending physician, and the interdisciplinary team daily.

Education and Experience

Experience Requirements:

• At least one year of clinical experience.

License Requirements:

• Current RN license in the practicing state.
• CPR certification.
• Valid driver's license, insurance, dependable vehicle or access to public transportation.
