Job Overview

Position Details

We are seeking a Registered Nurse (RN) Case Manager in the Dayton Area.

Responsibilities
  1. Lead the care team including LPNs, Aides, Social Workers, and Chaplains.
  2. Assess clients' physical and mental health, needs, preferences, and abilities to develop personalized care plans.
  3. Collaborate with clients, families, support networks, and healthcare professionals to implement care plans.
  4. Provide counseling or interventions based on clients' concerns.
  5. Document clients' progress, referrals, home visits, and interactions.
  6. Travel to service patients in their residences, including homes, assisted living, long-term care facilities, and hospitals.
Qualifications
  • RN License (Required)
  • Driver's License (Required)
  • BLS Certification (Preferred)
  • Experience in Hospice or Home Health (Preferred)
  • Knowledge of Hospice and Palliative Medicine (Preferred)
