Job summary

Spring Valley Hospice in Tupelo, MS is seeking a dedicated RN Case Manager to join our team and provide compassionate, patient-centered hospice care on site in Tupelo. You will assess needs, develop individualized care plans, administer treatments, and support families through difficult times.

The role emphasizes independent practice with strong team collaboration, adherence to hospice philosophy, and documentation excellence.

Qualifications

• Graduate of an accredited nursing program with current licensure to practice in the operating state.

Responsibilities

• Assess and monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with patient wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

As a RN Case Manager You will

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required , 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
