Job summary

Hospice Home Care in Russellville, AR is seeking a dedicated RN Case Manager to provide patient-centered hospice care and lead care planning. This role emphasizes independent work with team support, through regular visits and on-call coverage.

The RN will assess needs, develop aligned care plans, manage medications and symptoms, and educate families while maintaining thorough documentation and collaboration with the care team.

Qualifications

• Graduated from an accredited nursing program with current licensure to practice in the state of operation.
• One year nursing experience required; 3+ years preferred.
• Experience in hospice or similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile in good working order.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with patient wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
