Hospice Home Care is seeking a Registered Nurse Case Manager to coordinate patient care and support families in a hospice setting. You will perform comprehensive assessments and develop plans of care with the interdisciplinary team to improve outcomes.

The role requires an active RN license and at least 1 year of nursing experience, with 3+ years preferred, and hospice experience is a plus. Expect a compassionate, documentation-focused professional working in a collaborative environment.

Qualifications

• Current RN licensure in the state of practice.
• 1 year of nursing experience required, 3+ years preferred.
• Hospice or similar setting experience preferred.
• Knowledge of hospice philosophy of care.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients and families.
• Initiate patient plan of care and reevaluate as necessary.
• Utilize health assessment data to determine nursing diagnosis.
• Administer medications and treatments as prescribed by the physician in the plan of care.
• Counsel the patient and family in meeting nursing and related needs.
• Complete and maintain accurate clinical notes.
• Communicate with the physician and participate in interdisciplinary group meetings to meet medical and emotional needs.
• Ensure compliance with all state and federal legal and regulatory requirements.
• Additional responsibilities as assigned.
