Job summary

Adoration Health, an affiliate of BrightSpring Health Services, is seeking a Registered Nurse Case Manager for Hospice in North Carolina. As part of the team, you’ll provide patient-centered care in a home-based setting, coordinating plans and collaborating with an interdisciplinary team to improve patient comfort and outcomes.

Responsibilities include assessing patient needs, developing care plans, supervising nursing care, and educating families while maintaining meticulous documentation.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with insured automobile in accordance with state requirements.
• Possess and maintain CPR certification where required.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
