Job summary

Adoration Health, an affiliate of BrightSpring Health Services, seeks a Registered Nurse Case Manager for hospice care in Nags Head, NC. The role covers Dare and Currituck counties with a Monday–Friday 8am–5pm schedule and on-call rotation as needed.

The RN will assess patient needs, develop care plans, administer medications, manage symptoms, and educate families while collaborating with an interdisciplinary team. Experience in hospice is preferred and CPR is required.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
• Possess and maintain current CPR certification in states where required.

Responsibilities

• Assess/ monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
