Job summary

Haven Hospice in Gainesville, FL is seeking a Registered Nurse Case Manager to join our hospice team. You will provide one-to-one patient care in a supportive environment, managing plans of care and coordinating with a multidisciplinary team.

The role requires an active RN license, at least 1 year of nursing experience (3+ years preferred), and a commitment to quality documentation and patient-centered care.

Qualifications

• Graduate of an accredited nursing school with current licensure in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile insured as required.

Responsibilities

• Assess/ monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patient's wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient's family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch
