Job summary

Abode Hospice of Colorado seeks an experienced RN Case Manager to join our Colorado Springs team. You will provide 1:1, patient-centered hospice care, assess needs, and develop care plans during Mon-Fri, 8:00 AM – 5:00 PM scheduling, in a supportive environment.

Responsibilities include direct nursing care, pain management, family education, and on-call coordination as required by the local branch. Colorado license and CPR are required, with hospice experience preferred.

Qualifications

• Licensed Registered Nurse in Colorado with active license.
• 1 year nursing experience required; 3+ years preferred.
• CPR certification in states where required; hospice experience preferred.

Responsibilities

• Assess/monitor patients' physical, emotional, and psychosocial needs.
• Create hospice care plans aligned with patient goals.
• Direct nursing care: medications, treatments, interventions.
• Provide pain and symptom management.
• Educate and support patients' families and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

As a RN Case Manager You will:

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required , 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
• Possess and maintain current CPR certification in states where required.
