Job summary

Abode Hospice of Colorado is seeking a Hospice CNA to provide 1:1 patient care in the home setting. You will assist with bathing, dressing and mobility while observing patient conditions and reporting changes to the team.

The role includes helping with ambulation, medication assistance per state rules, emotional support, light housekeeping, and precise documentation. Strong communication and independence are essential in this collaborative hospice environment.

Qualifications

• Minimum six months experience providing personal care.
• Must be 18+ years old.
• Complete at least 75 hours of training with 16 hours practical.
• CPR certification required where states require it.
• Ability to read and follow written instructions.
• Comfort with hospice philosophy and terminally ill patients.
• Ability to work with minimal supervision.
• Strong communication and organizational skills.

Responsibilities

• Provide 1:1 personal care (bathing, dressing, mobility).

Responsibilities

As a Hospice CNA You will:

• Personal Care: Help with bathing, dressing, toileting, mobility, and repositioning

• Monitor & Report: Observe and report changes in the patient’s condition

• Mobility: Assist with ambulation, transfers, ROM exercises and safe equipment use

• Medication Assistance: Help with self-administered medications per state regulations

• Emotional Support: Offer companionship and comfort to patients and families.

• Light Housekeeping: Change linens, assist with meals, and maintain a clean space.

• Documentation: Maintain accurate and timely documentation

• Team Participation: Collaborate with hospice interdisciplinary team

Qualifications

• Meet the training requirements of the state of practice
• Minimum of six months experience including providing personal care
• Must be 18+ years of age
• Complete a minimum of 75 hours of classroom and supervised practical training, 16 of the 75 hours of classroom must be complete prior to a mandatory 16 hours of practical training
• Possess and maintain current CPR certification in states where required.
• Ability to read and follow written instructions and document care given
• Understands hospice philosophy, comfortable providing specialized care to the terminally ill
• Ability to work with little direct supervision
• Ability to handle death/dying
• Strong oral and written communication skills
• Good Organizational Skills
