Abode Hospice of Colorado is seeking a dedicated RN Case Manager to join our Colorado Springs team. This role focuses on patient-centered, 1:1 hospice care, coordinating plans with families and the interdisciplinary team. You will provide direct nursing care and educate caregivers to ensure comfort and dignity.

Qualifikationen

• Graduate of an accredited nursing program with active state licensure.
• 1 year nursing experience required; 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Must maintain CPR certification.

• Assess/ monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation


• One year nursing experience required , 3+ years preferred


• Experience in hospice or a similar setting preferred


• Knowledge of the hospice philosophy of care


• Commitment to clinical and documentation excellence


• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order


• Possess and maintain current CPR certification in states where required.
