Job summary

Assisted Home Health & Hospice in Los Angeles (Van Nuys area) is seeking a per diem Registered Nurse Case Manager to support our Home Health division across the San Fernando Valley, delivering about 10–15 visits per week.

You are a graduate of an approved school of professional nursing with an active California RN license, current CPR, health clearance, and the ability to complete competency exams. One year of nursing experience preferred.

Qualifications

• Graduate of an approved school of professional nursing.
• Current, active license in the State of California by the Board of Registered Nursing.
• Current CPR, health clearance and clinical skills self-assessment.
• Successful passage (80%) of Agency competency exam and core competency per skills checklist.
• Minimum of one year of professional nursing experience.

Responsibilities

• Deliver holistic, home health care to patients using the nursing process.
• Interact with patients, caregivers, and other health team members while maintaining standards of professional nursing practice.
